REGISTRATION DEADLINE
Return your completed application by April 28, 2011
Return completed, signed Application, fees, Parent Work Commitment sheet, Authroization for Treament sheet, and Apparel form at either Registration Night or to Eric Ezell, 2201 Mansfield Place, Lexington, KY 40515. Drop completed applications off no later than April 28. Entries received after April 28 will not receive the team T-shirt and run the risk of not getting preferred work assignments. Make checks payable to Hartland Swim & Dive Team or HSDT.
Late Registration: A late fee of $20.00 will be assessed for any late registration form and/or payments made after May 13, 2011.
